A dated public record of clinical or factual corrections. Typos that do not change meaning are not listed. The process sits in our editorial policy.
What counts as a correction?
A correction is logged when a page stated something factually wrong, out of date against its cited source, or dangerously incomplete — for example a missing 999 warning. We update the article and record the correction here. A source-check date records an editorial check; a medical review date changes only after a completed clinical review.
How do I report an error?
Email [email protected] with the URL and a short description. We treat these as a priority. Do not send personal medical details.

This log records corrections to published information. Service-link checks are not a new medical review of every article linked from that page.
